We have a transition worker in Telford and Wrekin.  The project was set up to work with young people, 14-19 years old who have a learning difficulty.

We aim to support, encourage and listen to young people so that they can develop the confidence to take a central part in their transition planning.

We will signpost parents and carers to find accurate, up-to-date information to help them support their young person to achieve their hopes, wishes and dreams.
